Japanese Alzheimer's Disease Neuroimaging Initiative
Japanese multicentre longitudinal study of 537 older adults with normal cognition, late amnestic MCI or mild Alzheimer's dementia, with structural MRI (1.5 T and 3 T), FDG and amyloid PET (PiB or BF-227), CSF, blood and cognitive data, run with protocols harmonised to ADNI.
Overview
J-ADNI is the Japanese counterpart of ADNI. It enrolled older adults at 38 clinical sites across Japan and followed them with structural MRI, FDG and amyloid PET, cerebrospinal fluid and blood sampling, APOE genotyping and a cognitive battery chosen to match the North American study as closely as possible. The goal was to establish imaging and fluid markers of Alzheimer's disease progression in a Japanese population and to support bridging of drug trials between Asia and the West. The data are held as controlled-access dataset JGAD000051 in the NBDC Human Database.
Composition
537 participants were enrolled: 154 cognitively normal older adults, 234 with late amnestic MCI and 149 with mild Alzheimer's disease, out of 715 volunteers aged 60 to 84 who were screened. People with Alzheimer's disease were followed for 2 years, the other groups for 3 years, with visits at screening, baseline and 6, 12, 18, 24 and 36 months. The release note states that data of all 537 participants are included. The dataset totals 310 GB.
Acquisition
Structural MRI used a 3D sagittal MPRAGE sequence modelled on the ADNI protocol, on 1.5 T GE, Philips and Siemens scanners, with phantom-based distortion correction. The file list also shows 3 T sessions. MRI comes as NIfTI, both raw and distortion-corrected, with FreeSurfer volume and thickness tables. PET used FDG and an amyloid tracer, either PiB or BF-227, delivered as ANALYZE images. Clinical, cognitive, CSF and blood tables use a data dictionary that is open to download.
Annotations
There are no manual image labels. Diagnoses at each visit, cognitive scores, quality control results for MRI and PET, and FreeSurfer outputs are included.
Known limitations
- The study start year differs between sources (2007 in a 2022 review by the principal investigator, 2008 in a 2017 MRI paper).
- Amyloid PET and CSF cover only part of the cohort: one longitudinal PiB analysis had 133 participants scanned at 11 PET sites, and CSF was taken only from participants who consented. Counts per modality are not on the NBDC pages.
- The MRI quality control manual describes PD/T2 scans, but the file list does not say whether they are released.
- Access is limited to the approved principal investigator and collaborators in the same organisation, and requires local ethics approval under Japanese guidelines.

NBDC Guidelines for Human Data Sharing, controlled-access data
Controlled-access data in the Japanese NBDC Human Database go to an approved principal investigator and collaborators in the same organisation, for the purpose stated in the application, after local ethics approval and a security checklist. No redistribution, no re-identification, yearly use reports, and deletion when the use ends.
Dataset JGAD000051 is controlled-access with Type I (standard) security.
