Interictal and postictal 18F-FDG PET/CT in epileptogenic zone localization

Brain FDG PET/CT of surgical candidates with drug-refractory focal epilepsy at a university hospital in Campinas, Brazil, acquired under EEG monitoring to compare postictal with interictal localization of the epileptogenic zone. 110 patients. Images not shared.

Overview

This cohort comes from the State University of Campinas (Unicamp) in Brazil. Patients with focal epilepsy that did not respond to drug treatment were enrolled prospectively between January 2015 and July 2017 and had brain FDG PET/CT while EEG was recorded around the tracer injection. Because some patients had a seizure shortly before injection, the study could compare how often postictal and interictal scans agreed with the reference location of the epileptogenic zone. The images are not shared: the paper has no data availability statement, and the SciELO figshare record attached to it contains only the article figures.

Composition

151 patients were scanned and 110 remained after excluding those whose epileptogenic zone could not be pinned down. The sample has 67 women and 43 men, aged 3 to 63 at inclusion (mean 35.0). Table 1 classes 64 patients as temporal and 46 as extratemporal lobe epilepsy. Ten patients had a single seizure in the hour before injection (postictal group), and 100 had none (interictal group). 24 patients went on to surgery; histopathology showed hippocampal sclerosis, cortical dysplasia, gliosis and single cases of oligodendroglial hyperplasia and glioma.

Acquisition

Patients rested for 90 minutes in a dimly lit EEG room, received 185 MBq of FDG, and were scanned 60 minutes later on a Siemens Biograph 40 TruePoint mCT with one 5-minute bed position over the head. CT served for attenuation correction and anatomy. EEG ran from 60 minutes before to 30 minutes after injection.

Annotations

Two nuclear medicine physicians read each scan independently and settled disagreements by consensus, marking hypo- or hypermetabolic areas as the presumed epileptogenic zone. The reference standard was histopathology with follow-up after surgery, or a multidisciplinary consensus based on EEG, video EEG, MRI and other tests.

Known limitations

  • Images, reads and clinical data are not released.
  • Single centre and single scanner.
  • Only ten scans are postictal.
  • Most reference locations rest on expert consensus rather than surgery.
