Porto Alegre PAH Cardiac MRI Cohort

Santa Casa de Misericórdia de Porto Alegre, cardiac MRI of patients with pulmonary arterial hypertension diagnosed by right heart catheterization

Cardiac MRI of patients with pulmonary arterial hypertension, used to compare right atrial area and right ventricular ejection fraction with catheterization, echocardiography, walk test and NT-proBNP markers. Images are not shared; only the paper's figures and tables are public.

Overview

This entry covers the cardiac MRI cohort of a retrospective single-center study of pulmonary arterial hypertension (PAH) at a referral center in Porto Alegre, Brazil, with authors from the Pulmonary Hypertension Group of Santa Casa de Misericórdia de Porto Alegre and UFCSPA. The authors measured right atrial (RA) area and right ventricular ejection fraction (RVEF) on cardiac MRI and tested how each relates to established prognostic markers: right heart catheterization (RHC) hemodynamics, echocardiographic RA area and TAPSE, six-minute walk distance and NT-proBNP. RVEF correlated more strongly with these markers than RA area did. The images are not public. The figshare record linked to the paper holds only its figures and tables.

Composition

The study includes 74 patients with PAH confirmed by RHC between January and May 2018, each with cardiac MRI within three months of the catheterization. Table 1 reports 61 women (82.4%) and a mean age of 42 years (SD 15). The causes were idiopathic PAH (32), corrected congenital heart disease (24), connective tissue disease (14), HIV (3) and portopulmonary hypertension (1). Forty-seven patients were in functional class I/II and 27 in class III/IV. Ten patients died during follow-up.

Acquisition

All scans were acquired on one 1.5 T Siemens Magnetom Aera. The protocol included a half-Fourier single-shot turbo spin-echo (T2-weighted) sequence, ECG-gated balanced steady-state free-precession cine in four-chamber and short-axis views, and a phase-contrast flow sequence orthogonal to the pulmonary artery trunk.

Annotations

A chest radiologist segmented the right and left ventricles on short-axis cine images with CardiacVX (GE Healthcare) and measured the RA area at its largest extent in the four-chamber view. Two radiologists reviewed every study, with a third resolving disagreements.

Known limitations

  • Images, segmentations and measurements are not shared, and the paper has no data availability statement.
  • Small retrospective cohort of patients already diagnosed with PAH, which the authors name as a selection bias.
  • Table 1 gives the RA area without a unit.
