Hepatic encephalopathy brain MRI radiomics (ISMETT)

Raw Data for the article: Brain magnetic resonance imaging radiomics features associated with hepatic encephalopathy in adult cirrhotic patients

Brain MRI of adult cirrhotic patients and non-cirrhotic controls from a transplant centre in Palermo, all on one 3T scanner, with texture features of the lentiform nuclei on T1-weighted images used to detect and grade chronic hepatic encephalopathy. 124 patients. Access restricted.

Overview

This retrospective cohort comes from IRCCS-ISMETT, a tertiary transplant centre in Palermo, Italy. Adults with liver cirrhosis and a control group without chronic liver disease had brain MRI with the same 3T protocol between October 2018 and February 2020. The study asked whether texture features measured in the lentiform nuclei on T1-weighted images can tell cirrhotic patients from controls and detect and grade chronic hepatic encephalopathy. The related Zenodo record holds the raw data for the article under restricted access.

Composition

124 patients were included out of 306 consecutive brain MRI exams screened: 70 with cirrhosis and 54 controls. Cirrhotic patients were 52 men and 18 women, aged 40 to 86 (mean 66.2); controls were 37 men and 17 women, aged 28 to 81 (mean 62.4). Cirrhosis was mostly viral (41), followed by NASH (11), alcohol (10) and other causes (8). 38 cirrhotic patients had chronic hepatic encephalopathy, graded by the West Haven criteria as grade 1 in 16, grade 2 in 16, grade 3 in 4 and grade 4 in 2. MELD scores are reported. Controls were scanned for other reasons, most often chronic hypoxic-ischemic encephalopathy (31) and migraine (11). Patients with a TIPS or with motion on T1-weighted images were excluded.

Acquisition

All exams ran on a GE Discovery 750w 3T scanner with a 32-channel head coil. The protocol had axial, sagittal and coronal fast spin echo T1-weighted images before and after gadobutrol, axial and sagittal T2-weighted images and axial FLAIR. T1-weighted slices were 4 mm thick with a 1 mm gap.

Annotations

A neuroradiologist rated T1 hyperintensity in the globi pallidi. A second radiologist, blinded to clinical data, drew a polygonal ROI in both lentiform nuclei on a single axial non-contrast T1-weighted slice at the level of the foramina of Monro, and 43 texture features were extracted with LIFEx.

Known limitations

  • Not openly available. The Zenodo files are restricted and the paper offers the data only on request. What the record contains (images or extracted features) is not described.
  • Single centre, single scanner, retrospective.
  • Controls are clinical patients with other neurological indications, not healthy volunteers.
  • Texture analysis covers one slice per patient.
