Methionine PET in lower-grade glioma (Humanitas)
Dataset related to article "Preoperative [11C]methionine PET to personalize treatment decisions in patients with lower-grade gliomas"
Amino acid PET/CT and brain MRI before surgery in adults with newly diagnosed grade 2 or 3 glioma, used to relate [11C]methionine uptake to tumour type, tumour extent and progression-free survival. 153 patients from one Italian centre. Data on request.
Overview
This retrospective single-centre cohort comes from IRCCS Humanitas Research Hospital in Italy. Adults with a newly diagnosed, histologically confirmed grade 2 or 3 glioma who had surgery between July 2011 and January 2021 were imaged before surgery with [11C]methionine PET/CT and MRI. The study asked whether methionine uptake predicts the tumour type under the 2021 WHO classification, adds to MRI in outlining the tumour and predicts progression-free survival. A Zenodo record linked to the article holds restricted files that can be requested.
Composition
153 patients were analysed. Of 201 patients who met the selection criteria, 48 were re-classified as glioblastoma under the 2021 WHO criteria and dropped. 93 had an IDH-mutant, 1p/19q-codeleted oligodendroglioma and 60 an IDH-mutant astrocytoma; 87 tumours were grade 2 and 66 grade 3. Median age at surgery was 40 years. Exclusions were treatment or biopsy before PET, missing precise grading and more than 100 days between PET and surgery. Molecular markers (MGMT promoter methylation, TP53, ATRX, Ki-67), extent of resection, adjuvant treatment and follow-up are reported.
Acquisition
[11C]methionine was produced on site, 300 to 500 MBq were injected and brain PET/CT started 10 minutes later on a GE Discovery 690 or a Siemens Biograph 6. Preoperative MRI ran on a 3T Siemens MAGNETOM Verio with T1-weighted images before and after gadolinium, T2-weighted and FLAIR images, and in most patients diffusion, functional MRI and DTI. Postoperative MRI at 48 hours and two months measured the extent of resection.
Annotations
Two readers rated each PET scan as hypermetabolic, isometabolic or hypometabolic, with two more resolving disagreements. Tumour-to-background ratios used a crescent-shaped contralateral background region. In positive scans a metabolic tumour volume was segmented in LIFEx at a threshold of 1.3 times background and compared with the FLAIR and contrast-enhanced tumour volumes after registration to MRI.
Known limitations
- Not openly available. The Zenodo files are restricted and released only on request; whether they contain images or only study tables is not stated, and the paper has no data availability statement.
- Single centre, retrospective, two PET scanners.
- Sex distribution is not reported in the paper's main text.
