Failed Total Hip Arthroplasty: Diagnostic Performance of Conventional MRI Features and Locoregional Lymphadenopathy to Identify Infected Implants

Pelvis MRI of patients with a failed total hip arthroplasty, read for periprosthetic findings and regional lymph nodes to tell infected from aseptic failure, using a metal artifact reduction protocol at 1.5 T. Only a spreadsheet of readings is public; images are not shared.

Overview

A retrospective cohort from the IRCCS Galeazzi Orthopaedic Institute in Milan of patients whose total hip replacement had failed and who were imaged with pelvis MRI before revision surgery. The study tested whether MRI signs around the implant, and the number and size of regional lymph nodes on the operated side compared with the other side, separate infected implants from aseptic failure such as loosening or polyethylene wear. The authors deposited a spreadsheet of their per-patient readings on Zenodo under CC BY 4.0. The MR images themselves are not offered.

Composition

119 patients, 66 of them women, with a mean age of 66.9 years (SD 12.4). In the public spreadsheet, 38 patients are marked as having an infected implant and 81 as aseptic failures, with the failure cause recorded for the aseptic group.

Acquisition

All patients had pelvis MRI at 1.5 T with a metal artifact reduction (MARS) protocol that included short tau inversion recovery and turbo spin-echo T1- and T2-weighted sequences. Scanner vendor and further parameters are given only in the full article, which was not openly accessible for this entry.

Annotations

Readers scored each scan for periprosthetic bone destruction, soft-tissue mass, effusion, synovitis, lamellated synovitis, extracapsular oedema, fibrous periprosthetic membrane, bone oedema and extracapsular collection or sinus tract. They counted and measured inguinal, obturator and iliac lymph nodes on both sides, from which the authors derived ratio and difference indices of node size and number. The spreadsheet also holds C-reactive protein, erythrocyte sedimentation rate, white blood cell count and the isolated microorganism.

Known limitations

  • Only tabular readings and clinical values are public; no images or segmentations are shared.
  • Single centre, single field strength and a retrospective design.
  • Some spreadsheet column headers and free-text entries are in Italian.
  • Details beyond the abstract (vendor, reader count, reference standard) come from the full article and are not recorded here.

The Zenodo record holds one spreadsheet with one row per patient: sex, age, inflammatory blood markers, infection status, failure cause, lymph node counts and sizes, and the presence of each MRI feature. It is openly downloadable under CC BY 4.0. No route to the MR images is published.
