Cerebellar ataxia FDG PET (Asan)

Different subregional metabolism patterns in patients with cerebellar ataxia by 18F-fluorodeoxyglucose positron emission tomography

Retrospective brain FDG PET of patients with cerebellar-type multiple system atrophy and spinocerebellar ataxia types 2 and 6, compared with crossed cerebellar diaschisis and healthy controls to map cerebellar subregional metabolism. 171 subjects from Seoul. Images on request.

Overview

This retrospective cohort comes from Asan Medical Center in Seoul, Korea. The authors used brain FDG PET to ask whether different cerebellar diseases leave different metabolic footprints across subregions of the cerebellum. Patients with cerebellar-type multiple system atrophy (MSA-C) and with genetically confirmed spinocerebellar ataxia types 2 and 6 were compared with two reference groups: patients whose cerebellar hypometabolism came from crossed cerebellar diaschisis after a frontal brain tumor, and healthy controls. The images are not public. The data availability statement offers the analysis data on request through a data monitoring committee at the hospital, and the PLOS figshare record contains only the three supplementary figures.

Composition

171 subjects in five groups: 44 MSA-C, 9 SCA type 2, 14 SCA type 6, 15 crossed cerebellar diaschisis and 89 healthy controls. Table 1 gives sex and mean age per group; across all groups there are 98 men and 73 women. The healthy controls, aged 17 to 85, were drawn from the hospital's normal FDG PET pool. The SCA cases cover every SCA diagnosis made between March 2006 and September 2013; the MSA-C patients were scanned between March 2006 and December 2010 and followed for at least two years. Clinical features, UPDRS III scores and CAG repeat counts come from medical records.

Acquisition

After at least six hours of fasting, 159 subjects were scanned on a CTI-Siemens ECAT HR+ (370 MBq FDG, 15-minute emission scan starting 40 minutes after injection, germanium-68 transmission for attenuation correction) and 12 on a GE Discovery 690 PET/CT with CT-based attenuation correction. Images were reconstructed with OSEM.

Annotations

A nuclear medicine physician drew 8 mm circular volumes of interest for 13 cerebellar subregions on a study-specific template built from spatially normalized control scans. Uptake was normalized to the occipital cortex, chosen by a voxelwise search for the least affected region.

Known limitations

  • Images and clinical data are not released; access depends on a request to the hospital.
  • Single centre with two scanner types.
  • MSA-C diagnoses are clinical, without postmortem confirmation.
  • Small SCA groups.
